According to the NIH: There happen to be different types of Priapism: Ischemic, Nonischemic, and Stuttering(the type Napoleon reportedly dealt with). All can cause problems if left untreated. And all come with severe pain - particularly the Ischemic.
- Ischemic (Low-flow) Priapism: This is the most common, painful, and dangerous type. It is caused by trapped, oxygen-deprived blood. It is a medical emergency requiring immediate treatment to prevent damage.
- Nonischemic (High-flow) Priapism: Often caused by trauma, this is less likely to be an immediate emergency, but still requires prompt medical evaluation.
- Stuttering Priapism: Recurrent, temporary, but painful episodes often associated with sickle cell disease.
NIH says:
If an erection lasts longer than 4 hours, get thee to an Emergency Room. After 4 - 6 hours tissue damage begins to occur. Things like Permanent ED, Tissue Damage and Fibrosis which is scarring and death of the smooth-muscle tissue, Deformity and Shortening.
However, if you have sickle cell: Get to an ER after 1 hour.
Depending on the severity, treatment can range from a simple injection of phenylephrine to draining the sludgy blood with a syringe to surgical shunts (last resort) which reroute the blood flow (the severity of which can be as simple as a needle to removing a portion of the Tunica.
Bottom line: Don't take unneeded --- or overdose on --- boner meds (the most common cause), and don't wear a shaft-only cock ring longer than 30 minutes. If you have an erection that will not resolve, get to an ER.
